Decision email says 'approved' for a refused request when the approver adds a note
Description
Northgate (SUP-820): a site worker got "Your request was approved" with the approver's note "Refused, ask your team lead first".
Steps to reproduce
- Refuse a request with a note.
Expected: "Your request was refused" and the note. Actual: the approved template, with the note.
In notifier:src/consumers/access-events.ts the note-present branch picks the approved template for both an approved and a declined RequestDecided.

Fixed and tested every event × note/no note. Looked back: 41 refused-with-note emails went out wrong since March. List to Ines so support can tell Northgate.
