Northgate: every approval decision in Q2, for our auditors
Description
For our auditors: every access request at our sites from 1 April to 30 June, who decided it, when, and what was granted. A spreadsheet is fine. We need it by 3 July.

Thanks. The auditors sampled 200 approvals: 37 have no reason recorded. They'll want a reason on every request from now on.

Hi Rachel, we'll pull it for you as a one-off CSV and send it on 1 July. Two things it can't show: a decision that was later changed shows only the last one, and it doesn't say which role someone held when they decided.
