Pellham: one-off export of all their data before they leave

Description

Pellham Transport leave on 31 July and want all their data (SUP-1011): people, depots, roles, every access request since 2024 with its decision. There's no export, so we write one by hand.

People, depots, roles and access are straight queries.
Request history is stored as events; flatten it to one row per request and decision.
One CSV per table, and a page explaining the columns.
Delivered through a link that expires after seven days.

Request history flattened and checked against the portal for 50 random requests. About four engineering days between Piet and me. If anyone else leaves we need this as a feature.
